Care Coordinator
 
We are currently seeking a Care Coordinator to join our Care Management Department. This position is responsible for managing patients’ physical and occupational therapy for compliance, medical necessity, patient progress, and appropriateness of discharge.
 
Position Hours: Monday-Friday (8am – 5pm)
Essential Functions:
 
  1. Manages therapy for compliance, medical necessity, patient progress, and appropriateness of discharge
  2. Processes evaluations, patient progress summaries, and manages patients’ physical therapy
  3. Facilitates the patient’s achievement of maximum potential in skilled therapy
  4. Provides weekly or bi-weekly Progress Notes to clients
  5. Maintains diary compliance
  6. Ensures timely follow up with internal and external customers
  7. Consistently meets departmental QAQI thresholds in quality and service
  8. Provides clients and patients with exceptional service
  9. Intake of patient referrals as needed
 
Qualifications:
 
  1. High school diploma
  2. Minimum 2 years customer service experience
  3. Strong organizational skills and ability to multi-task
  4. Excellent telephone skills
  5. Basic computer skills
  6. Medical terminology (preferred)
